feat(session): SESSION-DURABILITY-b-KN Hub continuity (BV pass)

Hosted type:session admission with SESSION_EXPIRED/INVALID, 3h–24h HUB_JWT_EXPIRY integer seconds, browser/CLI establish-refresh delivery, and Hub fresh-session helper for copy + agent credentials.
