feat(hosted-mcp): R2 folder listing on hosted vault resource template

Non-.md knowtation://hosted/vault/{+path} returns JSON from the same canister list as list_notes (folder + limit/offset). Updates tests and parity/handoff docs; deeper pages remain list_notes until optional URIs.
