fix(gateway): relax MCP OAuth rate-limit X-Forwarded-For check behind Nginx

express-rate-limit in @modelcontextprotocol/sdk OAuth routes throws ERR_ERL_UNEXPECTED_X_FORWARDED_FOR when Nginx sets X-Forwarded-For. Pass validate.xForwardedForHeader: false for SDK token/authorize/register/revoke limiters so Cursor OAuth token exchange succeeds on mcp.knowtation.store.
