fix(hub-ui): warn when localhost uses wrong API base; docs for GitHub OAuth callbacks

- Banner + clear localStorage hub_api_url when localhost tab calls non-local apiBase - Settings Backup shows API base on localhost for debugging - CONNECT-GITHUB §0: copy-paste table for login vs github-connect vs bridge - Gateway README: post-deploy CORS, BRIDGE_URL, SESSION_SECRET checks - HOSTED-HUB-VERIFY: clarify check:gateway-cors ok means CORS configured - hub/README + index.html: stress both GitHub callback URLs for local Hub - Bump hub.js cache buster
