fix(search): scope approval_logs to approvals/ path in ANN + widen KNN

Approval logs only previously post-filtered global top-k, so thin log chunks often never appeared. Pass folder=approvals into vector search, raise KNN cap when a path-prefix filter applies, and wide-fetch for notes-only scope before excluding approvals/.
