fix(gateway): Netlify-safe import.meta guard in breached-passwords (INF-KN-0)

Gateway cold-start crashed on api.knowtation.store when Netlify CJS bundle stripped import.meta.url from hub/lib/breached-passwords.mjs. Use the same libDirname() fallback pattern as hub-proposal-rubric.mjs.
