fix(bridge): clarify hosted embedding failures and validate URLs

- Normalize and validate Ollama embed base URL (lib/embedding.mjs); trim provider/model. - Bridge: trim EMBEDDING_* env, log resolved provider once per cold start (no secrets), and expand bare Invalid URL errors with operator hints. - Gateway: validate BRIDGE_URL at startup (http/https). - Docs: DEPLOY-HOSTED bridge indexing; bridge and gateway README embedding notes. - Tests: normalizeOllamaEmbedBaseUrl unit tests.
