feat(hosted-mcp): Track B2 prompts — five registerPrompt handlers, ACL, tests, docs

Add meeting-notes, knowledge-gap, causal-chain, extract-entities, and write-from-capture on the gateway with the same bridge/canister paths as existing tools. write-from-capture requires editor role; causal-chain uses search chain filter plus canister reads (documented vs local graph).

Extend mcp-hosted-prompts tests, verify checklist copy, and update parity/interlock/playbook/handoff docs.
