Phase 11.1: Hub first screen (login) + plan order/status

- IMPLEMENTATION-PLAN: add 'Phase 11.1 and follow-on' table (order and status); mark Hub first screen Done; next = Phase 13 invite or landing. - Login panel: hero (title, tagline, intent), CTA block (sign in above, How to use secondary); #app gets .login-screen when login shown; header provider buttons emphasized on login screen.
