feat(cli): add knowtation doctor for vault and Hub health

doctor loads local config, checks vault readability, optionally probes KNOWTATION_HUB_URL /health and notes list with token. Human output includes WHY-KNOWTATION token layers; --json emits machine-readable checks. Document in README and RETRIEVE reference; extend CLI tests with isolated Hub env.
