hub: import destination parity with New note and aligned hints

- Wire import modal to same project/folder/path controls as full create (facets, subroots, output_dir on import API and import-url JSON). - Match Path and Subfolder helper copy on both panels; bump asset cache query strings. - Extend create-path label/hint styles to #import-create-path-block. - Document in web/hub/CHANGELOG.md.
