fix(gateway): MCP canister user matches Hub effective id

- mcp-proxy: pass effective_canister_user_id from hosted-context as canisterUserId - mcp-hosted-server: use canisterUserId for all canister X-User-Id; fix vault-info scope; document in AGENT-INTEGRATION - tag_suggest: default neighbor sample 40, optional neighbor_limit 5-80; local runTagSuggest parity - Tests: mcp-hosted-canister-user-parity (list_notes, get_note, write, export, vault-info); tag_suggest neighbor_limit
