chore(muse-hygiene): shared muse-local-setup.sh + canonical vault hygiene note + rc15 bridge-deletion issue re-file

Prevents recurrence of the .env/config/local.yaml deletion (root cause: raw 'muse bridge git-export --git-dir .'; still present in muse 0.2.0rc15). Adds per-clone skip-worktree setup, canonical guidance note, and upstream issue draft.
