fix(hub): vault switcher UX, settings server view; docs parity snapshot

- Vault switcher: show when 2+ vaults; tooltip instead of inline hint - Settings → Vaults: server view (user ID, allowed vaults, data dir) - PARITY-PLAN: Phase 3 = hosted multi-vault; Phase 4 done - STATUS-HOSTED-AND-PLANS: §2.1 parity snapshot table; bridge embedding note - Bridge README: OLLAMA_URL must be real API, not ollama.com
