fix(hosted-mcp): relate titles never stuck at null

- Empty catch left r.title unset when canister GET failed (upstreamFetch throws) - titleFromMarkdownBody required '# ' so '## Heading' never matched - Always fall back to titleFromPathStem(r.path); catch uses same stem - parseCanisterFrontmatter: exclude typeof null object branch
