feat(hosted-mcp): add cluster tool and bridge POST /api/v1/embed

Register hosted MCP cluster (viewer+): canister note list/get with folder/project filters, cap 200 embedded notes and 2000 list rows, k-means on bridge document vectors. Add bridge batch embed route using resolveHostedBridgeContext (same auth model as POST /api/v1/search) and embedWithUsage with document vectors (same config as POST /api/v1/index). Update golden tool lists, docs inventory, and mcp-hosted-cluster tests.
