fix(hub): multi-vault sqlite-vec isolation, search UX, bridge vault scope

- sqlite-vec: store vault_id, filter search by vault_id, widen KNN when scoped; migrate vec0 table without vault_id via DROP+recreate - indexer: composite point ids (vaultId::chunkId) to avoid cross-vault collisions - bridge: same composite ids + vault_id on upsert; pass X-Vault-Id on search - hub API: drop semantic hits whose path is not a file in the active vault - lib: noteFileExistsInVault for reuse + tests - Hub UI: Clear search, scoped semantic POST body (project/tag/folder/dates), match-strength line, hint copy, cache-bust hub.js - tests: vault_id isolation regression for sqlite-vec; noteFileExistsInVault
