feat(2G-d-b): task + task-loop write proposals (SD-4 facade, gates off)

Implement typed propose/apply facade over /proposals for tasks and loops: Hub/CLI/MCP routes, approve dispatcher hook, seven-tier tests, hosted smoke. TASK_WRITES_ENABLED remains off until Tier 3.
