feat(gateway): hosted MCP hub_create_proposal via gateway API

- Register hub_create_proposal when gatewayApiBaseUrl is set; POST /api/v1/proposals with JWT and X-Vault-Id so billing and proposal-create augmentation run like the Hub UI. - Extend mcp-tool-acl with evaluator (same write-class tools as editor, incl. hub_create_proposal) and ROLE_RANK for prompts. - Self-hosted Hub: allow evaluators to POST /api/v1/proposals (parity with MCP). - Tests: tools/list golden sets, mocked hub_create_proposal, ACL cases. - docs/AGENT-INTEGRATION: hosted MCP proposals for editor, evaluator, admin.
