fix(hub): prevent duplicate detail actions from concurrent openNote fetches

- Track hubOpenNoteSeq and ignore stale API completions in openNote. - Clear detail-actions before attachNoteDetailReadActions on successful load. - Add contract test for the stale-response guard.
