feat(mcp-proxy): env MCP session TTL and max sessions (defaults 8h / 8)

Hosted MCP uses in-memory sessions; extend default idle TTL from 30m to 8h and raise max concurrent sessions 5→8 to reduce Cursor disconnects.

- MCP_SESSION_TTL_MS (ms, clamped 5m–24h) - MCP_MAX_SESSIONS_PER_USER (clamped 2–20) - Export parseMcpSessionTtlMs / parseMcpMaxSessionsPerUser for tests - Docs: AGENT-INTEGRATION, DEPLOY-HOSTED matrix, NEXT-SESSION R1/R2 note
