feat(hosted-mcp): R1 vault note ResourceTemplate (knowtation://hosted/vault/{+path})

Register hosted-vault-note when get_note is allowed; same canister GET and headers as get_note. Markdown paths only; reject traversal. Add tests and parity matrix row.
