fix(hub): import evaluator-may-approve from hub/lib (self-hosted npm run hub)

hub/server.mjs must use ./lib/... to match hub/lib/hub-evaluator-may-approve.mjs; ../lib/ pointed at repo root and broke local Hub startup.
